Schedule for 1st Interview (A&E and admissions ward)

Section (i) Re experience in A&E

1. Did you know what to expect when you first arrived in casualty?

2. How long were you waiting in casualty before you were seen?

3. When the staff asked you about your illness/injury, did you get a chance to tell them all you wanted them to know?

4. Did you need to have any tests or procedures carried out in casualty?

5. Were you kept informed about the results of any tests/procedures you had?

6. Did you need to have any treatment in casualty?

7. Did the doctor ask you how you felt about the treatment?

8. When did you find out you were going to be admitted?

9. After you were told you were being admitted, how long did you have to wait for a hospital bed to become available?

10. Were your relatives kept informed of what was happening to you while you were in casualty?

11. Were you given any written information in casualty?

12. What other information would you like to have been given while you were in casualty? Section (ii) Re experience in admissions ward

13. How long did you wait in this ward initially before you were seen by a nurse?

14. Do you know what to expect during your stay on this ward?

15. Have you been seen by a doctor since you were moved to this ward?

16. (If has not yet seen a doctor) have you been told when a doctor will come and see you?

17. Have you had any more tests or procedures carried out since you arrived on the ward (or are there any planned)?

18. Were you told the results of these tests/procedures?

19. Have you been kept informed about your condition since you moved to the ward?

20. Will you be having any treatment while you are in this ward (or have you had any treatment)?

21. Did the doctor ask you how you felt about this treatment?

22. Have you been given any written information since you arrived on the admissions ward?

23. What other information would you like to have been given?

24. Will you be moved to another ward at some stage?

Schedule for 2nd Interview (General medical or surgical ward)

1. Did you know what to expect during your stay on this ward?

2. Have you been given any new medications since you came to the ward?

3. Have you had any more tests or procedures carried out since the last time I saw you (or are there any planned)?

4. Has a consultant come to see you since you moved to this ward?

5. Did the consultant ask you how you felt about having the treatment/operation?

6. (If had operation) Did an anaesthetist (the doctor who gives you drugs to put you to sleep before an operation) come to see you on the ward before your operation?

7. Were you given as much information as you wanted about your treatment/operation?

8. Did you know what to expect after your treatment/operation?

9. Were you told how the treatment/operation went?

10. Will you be discharged home soon?

11. Were you given any information to prepare you for going home?

12. Were you given any written information during your stay on this ward?

13. Was there any other information you would like to have been given during your stay on this ward?

Schedule for 3rd Interview (After discharge, in patient's own home)

1. When did the staff tell you that you would be going home?

2. Did the staff explain your medications to you before you were discharged?

3. Did the staff advise you how to look after yourself when you got home?

4. Were you told what to expect or look out for when you got home (e.g. pain, bleeding)?

5. Were you told who to contact if you had any complications?

6. Did the staff talk to your relatives about the care you would need when you got home?

7. Did the staff ask you if you needed any help at home e.g. with dressing?

8. Did the staff discuss with you if you needed any aids or equipment (or adaptations) at home?

9. Did the staff reassure you about how you would manage when you got home?

10. Did the staff tell you about any voluntary organisations or charities that might be able to give you help or support relating to your condition?

11. Before you went home, did a nurse arrange an outpatient appointment for you?

12. Did the staff tell you they would contact your GP to let her/him know that you had been discharged

? 13. Did the nursing staff contact your relatives to let them know when to collect you?

14. Did a nurse ask if you needed transport organised to take you home?

15. Were you given any written information before you were discharged?

16. Were you satisfied with the information you were given before you were discharged?

17. If the staff said they would make arrangements for you (e.g. organise help at home), did they do this?

18. Was there any other information you would like to have been given before you were discharged from the hospital?
